•	Bereavement Coordinator, assist patient’s and family members with grief counseling and end-of-life planning
•	Access and assist with community resources for patients and others involved with their care
•	Participate in Interdisciplinary Team (IDT) meeting to initiate, review, and update patients’ plan of care with other hospice team members

•	Ensure that all components of the care plan are being followed by reviewing MDS’s and nurses’ notes and attending a weekly Interdisciplinary Team (IDT) meeting with other department managers
•	Discharge planning- providing information and community resources related to a patient’s care needs
•	Schedule and attend care conferences with patients and others involved with their care
•	Participate in a monthly Psychotropic review meeting with contracted pharmacist and nursing manager
•	Strong understanding of Medicare, Medicaid, HMO’s and private insurances
•	Maintain confidentiality of all patient care information in accordance with HIPAA guidelines
•	Daily online charting
•	Community referrals as needed- home health, DME, hospice and palliative care, establishing Primary Care, Medicaid services- and serve as a link between the patient and these resource systems
•	Counseling residents and their families
•	Ensuring the social and emotional needs of each patient are being met
